Hello,
Please find attached the execution version of the CBI Addendum to the Electronic Service Provider Agreement for signature by your agency (HSP). The Addendum authorizes CAMH as your PHIPA agent to provide your CBI data to the Institute for Clinical Evaluative Sciences (ICES) for permitted purposes. Only HSPs that sign and return the Addendum prior to Friday March 27, 2015 will have their data included in the first upload to ICES on March 31, 2015.
As mentioned previously, the Addendum reflects the recommendations and decisions made by the CBI Privacy, Security and Data Access Sub-group, and the CBI Working Group. The Addendum has been reviewed by ICES, CAMH and independent counsel who has reviewed the Addendum on behalf of the HSPs. Please find attached the final version of the Addendum for signature. We request that the authorized signatory (or signatories) for your HSP complete, sign and return Schedule A (the Addendum 1 Adhesion Agreement) by March 27th at the latest to ensure that your data is uploaded in time for the March 31st deadline.

The Addendum 1 Adhesion Agreement will only come into effect upon the signing of the Data Sharing Agreement (DSA) among ICES, CAMH and Reconnect (as CBI agent for the HSPs) which is anticipated for early next week.
In order to activate the Addendum, please follow the steps below:
- Print a copy of the Addendum and review.
- Have an individual with the authority to bind the HSP complete and sign SCHEDULE A (p.6) of the Addendum and return the ENTIRE document, using one of the ways below, to the CBI Support Centre. SCHEDULE A is the only section that requires a signature and the completion of information as indicated below.
