The report outlines findings from a preliminary investigation into when and how clients interact with both the community-based care and acute care sectors. Initiated in 2012 and with funding from the Toronto Central LHIN, CBI has begun to yield new insights into the client journey, length of stay, and geography of service utilization within the community-based health services sector.
